A resellers tax identification number is required. If accepted as a professional member you will receive wholesale pricing of 40% off retail on all LifeSpa products. Please take these steps to become a Wholesaler:
1. Fill out and submit the online form: http://www.lifespa.com/registration.asp
2. Fax a copy of your reseller’s license to 303-530-4409.
3. Wait for an email from LifeSpa, confirming your application has been approved.
4. Begin ordering online. The minimum order is $150.00
Please Note: If you are from Oregon or New Hampshire, which does not have a reseller's tax identification number, please fax us a copy of your professional license and a description of how many patients you see per week and how you plan to prescribe our products.
